 Participant Waiver and Release of Liability
In partnership with The V Foundation Event Dates: July 31, August 1, and August 2, 2025
Location: The Estate Yountville, Napa CA This Participant Waiver and Release of Liability (the "Agreement") is entered into by the undersigned participant ("Participant") in favor of Live Well Lead Well, its owners, officers, directors, employees, volunteers, affiliates, sponsors, and The V Foundation for Cancer Research (collectively, the “Released Parties”). 1. Voluntary Participation:
I acknowledge that my participation in the Sunrise Full Body Flow is voluntary. I understand that this is a physical wellness activity involving stretching, breathwork, bodyweight movements, and meditation. I certify that I am physically fit and have not been advised otherwise by a qualified medical professional. 2. Assumption of Risk:
I am aware that participation in any physical activity may involve risks including, but not limited to, musculoskeletal injury, dizziness, fainting, or other health risks. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Released Parties. 3. Medical Release:
I understand that no medical services will be provided on-site. I consent to receive emergency medical treatment if needed and agree that I am responsible for any associated costs. 4. Waiver and Release of Liability:
In consideration of being permitted to participate in the event, I hereby release, discharge, and hold harmless the Released Parties from any and all liability, claims, demands, or causes of action that may arise from my participation in the event, including any injuries or illnesses, property damage, or other losses, whether caused by negligence or otherwise. 5. Media Release:
I grant permission for Live Well Lead Well and The V Foundation to use photographs, video, or audio recordings of me taken during the event for promotional and marketing purposes without compensation. 6. Governing Law:
This Agreement shall be governed by and interpreted in accordance with the laws of the State of Michigan. 7. Entire Agreement:
This document represents the entire agreement and supersedes all prior discussions or representations. If any portion is found to be unenforceable, the remainder shall remain in full force and effect.
